Coleridge 

1).Difference between Poem & Poetry:-
Poem is created work & poetry has particular hidden meaning in it.
According to Coleridge,imagination is more important in writing.one has to agony & pain during the writing.
To differentiate both i would say that,with pain,feelings when one starts his thinking at that time,the way of thinking,ideas that going on his mind is an activity,its called Poetry.& when it comes out with conclusion is called Poem.
For example:-If one have so many ideas regarding social problems & at the end he will gave a name of that thoughts,a name in a particular structure like Crime.so here,thoughts regarding social problems is poetry & crime is here poem.

2)  Different between Poem and Poetry

     Poetry and Poem both are much different but both are representative of each other. Poetry is a skill it is a art of to make Poem. Poem is made by Poet’s imagination his or her feeling and knowledge of wording and this kind of creativity we can say a pure talents to make Poem is called Poetry
.
     Example : In a classroom teacher teach to the students by his of her knowledge and effort and what is the output ,output is knowledgeable student. Here knowledgeable students  are  Poems skill of teacher is Poetry. Poetry is the art to give beautiful out in form of Poem.
